What to expect in your initial visit at the
Rinehart Center for Reproductive Medicine….
At your first appointment with Dr. Rinehart you
can expect to spend at least forty-five minutes with
the physician in a detailed, comprehensive
consultation. This usually includes a review of any
previous medical records and a discussion of
possible diagnoses and treatment options. Sometimes,
if appropriate, the physician may suggest doing
simple diagnostic testing at this time, such as a
blood test or an ultrasound.
We encourage you to bring your spouse or partner
to your initial visit, but this is not required. You
will have time to ask questions or express concerns
with the physician and meet the nursing staff. Our
administrative staff will also be available at this
time to help you with financial and other practical
non-clinical questions.
What information can you bring to your initial
visit?
Before your appointment date, we will send you an
information packet. Please fill out the female and
male (if appropriate) questionnaires and bring the
completed forms with you to your initial visit. We
also ask that you bring (or send ahead of time) any
medical records from other health care providers
that you have seen that may be relevant to your
visit with us.
If you have had previous fertility treatment at
another center, please have your medical records
sent to us at least one week prior to your scheduled
appointment.
We also ask that you bring your current insurance
card or cards, any referral forms that your
insurance carrier may require, and the signed copy
of the Patient Responsibility Form that was also
included in the packet form our center.

Please bring your completed forms in with you on
your initial visit.
